Med3 Notes

If you need to request a Med3 fit note from the GP for your employer, remember you must self-certify for the first 7 days of your illness by filling out Form SC2 from the HMRC Website. Follow this Link to the SC2 form here.

The GP issues the Med3 to coincide with the expiry of the self-certification but please remember the doctor does not backdate from the date they see you.

If your note is for the Job Centre to assist with your benefits, please speak to reception as it may not be necessary that you have an appointment to request a continuous note. Please note: GPs cannot give you a Med3 for a period of time when you have not been seen by the GP, nor will GPs back date Med3 to cover a period of illness that you have not been consulted for.
