Summary Care Record
The Summary Care Record (SCR) is a record held centrally (electronically) on the National NHS Spine, and contains information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had to ensure those caring for you have instant access to information to treat you safely. This means that if you have an accident or become ill, healthcare staff external to the Practice treating you will have immediate access to important information about your health. Your GP practice is supporting Summary Care Records and as a patient you have a choice:
If you do nothing we will assume that you are happy with these changes and create a SCR by default.
Named GP
There is a contractual requirement to provide all patients with a named accountable GP. Your named GP will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice but it will mean you have been allocated a named GP by default to allow patients to be shared equally amongst the GPs at the Old Fire Station.
As a patient you are entitled to know who your named accountable GP is, and if you have been elected one by default you can change your Named GP if you are not happy, to a doctor at the practice which you are more known to. You can do this over the phone or by asking a receptionist on your next visit.
